I do both. Generally I work out the basic composition and perspective on paper first. Depending on how complex a picture's going to be, I'll sometimes draw the whole thing on paper first. I don't have a scanner, and I would take a picture with my camera but the battery's dead and I keep forgetting to recharge it. So I just copy it by sight. And then (like I did with the 2nd and 3rd pictures) I go over the finished drawing on another layer two or more times trying to simplify it.
